If you are applying in England, Wales or Scotland, please request a Disclosure application form by calling (020) 7251 3448 or e-mailing to camps@bunac.org.uk (please include your address). Please send your completed Disclosure application forms, together with the rest of your application, in the same envelope. This only applies to returning Summer Camp USA applicants.
